This appointment-only clinic helps eligible Hillsborough County K–12 students complete school-entry health requirements. Licensed medical partners provide school physicals, routine immunizations, immunization-record review, and help updating or obtaining Florida’s DH 680 school immunization form.
Children must be uninsured, underinsured, or enrolled in Medicaid and need qualifying school-entry services—for example, entering kindergarten, entering seventh grade and needing the required Tdap booster, or enrolling in a Hillsborough County public or charter school for the first time. Children with private or commercial insurance are not eligible.
Before the appointment
- Create a family account, register every child who needs care, and select an appointment.
- Upload current immunization records and bring any school forms supplied to the family.
- A parent or legal guardian must attend or complete the required consent steps.
- Most visits take approximately 45–90 minutes. Walk-ins cannot be served.
Medical services are provided at no cost, but reserving an appointment requires one nonrefundable $10 administrative fee per family, not per child.
